Scholarship Application Process

Kids’ Chance of Ohio provides college scholarships to the children of Ohio workers who have been severely or fatally injured in a workplace-related accident. We award as many scholarships as the current year’s funding will allow.

What is the application process?
  • An applicant (the child of fatally injured or permanently and totally disabled worker) or other interested party may contact Kids’ Chance of Ohio or a Kids’ Chance of Ohio Board Member to inquire about scholarship availability and the application process.
  • Kids’ Chance of Ohio will provide a scholarship information packet including the following: Kids’ Chance Scholarship Fact Sheet, Kids’ Chance Scholarship Application Portal Link.
  • The applicant and his/her parent/guardian/representative must complete the application using the Application Portal to submit the completed application along with additional supporting documents to Kids’ Chance of Ohio. Please see the application Fact Sheet for required supporting documentation. 
  • A preliminary review of the application by Kids’ Chance of Ohio Board will be made to determine the completeness of the Application. If the Application is not complete, additional necessary information will be requested of the applicant. The Application will not be considered as received until all necessary information is received.
  • The Kids’ Chance Board will request that the Ohio Bureau of Workers’ Compensation provide verification of the eligibility of the applicant.
  • Once the eligibility of the applicant has been verified by The Ohio Bureau of Workers’ Compensation, Kids’ Chance Board will perform a review of the application and will confirm the accuracy of information provided.
  • Kids’ Chance Board of Directors will consider all applications of confirmed eligible applicants and, after reviewing the students’ applications, will make a final determination as to whether an award is to be made and the amount of the award. Scholarships are awarded for an annual period, (either by academic or calendar year).
  • Kids’ Chance of Ohio Board of Directors will notify the applicant as to his/her award and the procedure to be followed to receive the scholarship award amount.
  • The scholarship recipient will be required to provide Kids’ Chance with timely documentation of satisfactory academic/educational performance (quarterly or semester grade reports) in order to continue to receive the scholarship award.
Who is eligible for a scholarship?
  • The applicant must be the child of an employee who has been declared to be permanently and totally disabled or who was fatally injured as a result of a work-related injury or occupational disease.
  • The death or injury or illness must have occurred as a result of work activities performed for an Ohio employer covered by the Ohio workers’ compensation law.
  • The injury or death must have resulted in a decline in the family’s income.
  • The applicant must be between the ages of 16 and 25.
  • The applicant must be enrolled full time in a trade or vocational school; in industrial/commercial training; at a junior college or community college; in an undergraduate program at a college or university; or in a graduate program at a university.
  • While scholarship awards are intended for Ohio residents, they may be used for institutions outside of Ohio as well as in state.
  • Scholarships will not be awarded to individuals who are directly related, either by birth, adoption, or marriage, to members of Kids’ Chance of Ohio Board of Directors.

What are the guidelines for applying for a scholarship?
  • Applicants must submit a scholarship application form with all supporting documents.
  • Applications must be submitted no later than one month before the beginning of the semester or quarter.
  • Scholarships are awarded on the basis of a student’s need and the funds available to Kids’ chance of Ohio. The maximum scholarship award is $5000 per year.
  • Scholarships may be used to pay tuition/fees, books, and room and board.
What documentation is required?

The following documents must be included with the scholarship application:

  • Transcript from the applicant’s last two years of attendance at an academic institution.
  • Financial aid applications and replies (including FAFSA and SAR documents) from college or technical school.
  • Two letters of recommendation.
  • Industrial Commission order approving death claim or proof of acceptance of death claim from a self-insuring employer (if applicable).
  • Industrial Commission order declaring parent permanently and totally disabled or proof of acceptance of permanent and total disability from a self-insuring employer (if applicable).
  • Renewing Applicants need to submit up-to-date transcript and financial documentation noted above as well as the current year’s Student Aid Report (SAR) documents and an explanation of any extenuating or mitigating circumstances relevant to their application for more financial aid.
What types of educational institutions and expenses can the scholarship utilize?
Scholarship awards can be used for the following types of institutions:

  • Trade School/Vocational School
  • Industrial/Commercial Training
  • Junior College/Community College
  • College Undergraduate
  • College – Graduate School Scholarships may be used for both public and private educational institutions, both within and outside of the State of Ohio.

Scholarships may be used for the following:

  • Tuition/Fees
  • Books
  • Room and Board
